MFA Issues For Some Users

For some of my users, they are using the MS Authenticator app, adding a new account via the "+" sign and then selecting Other as account type. At this point, they are scanning the QR code presented at their login screen, but the 6-digit code does not appear on the home screen of the Authenticator app. There is a 6 digit code, but only if you click into the account on Authenticator. Needless to say, when they try to login via Web Browser, they are still seeing the option to scan the QR code and the 6-digit code from the app is not working. What am I missing here?

@scottp scanning the QR code adds the account to the authenticator app. Is the account not getting added to the authenticator app?
