user profile data missing

We are having some issues where the users profile data has gone missing, I have run the profile changes report in travel and it does not show that anyone has manually changed or deleted the information. The strange thing is that if he had not added this originally he wouldn't have been able to book a trip which he has done, has anyone else seen this? This is not the first time we have had this happen to multiple users in the past also.
