Mileage Calculator - Route cannot be calculated

My company requires that I use the mileage calculator for submitting reimbursement for travel in a personal vehicle for work. 

Recently it doesn't matter which browser or computer I use, I almost always get the error:
' Route cannot be calculated. Either one of the waypoints is incorrect or the route is not drivable '

I'm happy to try any tips or suggestions to resolve this. The only current work around I have is using the mobile app to submit my mileage.

Thanks for offering to take a look! I did sent you the addresses, but it doesn't seem to matter what addresses I put in, nothing is calculating.
