Password Issues

User has stated that his provided credentials will work, and then after several weeks they will no longer work
on his attempt to log in. Is there an application log that can be investigated for what might have occurred to ensure the password was not changed by someone else?

@Linzy_234 I did find in Cognos that there is a data item labeled Password Change Date/Time. However, it doesn't appear there is a way to see who changed it. At least it could help narrow down if it was this employee or not who changed the password. 

Are you familiar with Cognos reporting? If so, the data item is found here: Lists > Employee Information > Additional Details. You'd have to add the data item of Employee Name first,
