Empty receipt after receipt is applied

Travelers are finding that after attaching a receipt to a charge, that the system holds a line under Available Receipts.  The line is not valid, it shows the information that pertains to the receipt attached, but the image is blank.   This is not a consistent issue. 

 

Has anyone experienced this?
