Analytic Reporting

We need a report for failed items with Expense category, failure amount/currency, failure reason and the USD(reporting currency) amount for the failure item.

I don't believe this data is available in our Analytics reporting. You might want to reach out to our Consultative Intelligence team about your customized reporting needs. Reach out to your SAP Concur Client Success Manager, they should be able to provide you with the contact information for the Consultative Intelligence team.
