Can you please try below steps and let me know if it works for you as expected.
1. Create report and add columns/data objects as per your requirement including "Approval Status" column.
2. Now click on "Approval Status" header field as shown below, click on 3 dots and select "Build Prompt Page" option. It will automatically generate prompt page for you as shown below in 2nd screenshot.
3. Now save the report and Click on Run.
4. When you run, select required "approval status" from the drop down list and click finish.
