Auto Approval of expenses

The workflow has an external validation (API) and then manager approval. There are times when the manager approval is happening automatically after the response from webservice. On client side the webservice response is successful but Concur logs show the API has 500 error. 

I assume the error is at the concur workflow guidelines and when they get the internal 500 error for not able to capture the response or any other data. Do anyone have this issue in the past?
